Need help applying for Medicaid?

The Louisiana State Medicaid application can sometimes feel overwhelming to complete.
At ABC Pregnancy, we have trained staff to walk you through the process of signing up and answer the questions you may have about the financial costs of prenatal care and delivering a baby.

This service is by appointment only, so please contact us through the contact tab to set up an appointment.

What will you need to apply for Medicaid?

INFORMATION FOR MEDICAID APPLICATION APPOINTMENT

• Picture ID

• Social Security number for you and any other members of your household

• Verification of income for the last month. If working, we will need a pay stub

• If you are married we will need his stub as well

• If you receive child support and/or alimony, we will need those totals

• Bring any current health insurance: name, policy, and ID numbers

• If in house applying- Please allow up to 1 hour for your appointment

• Licencia de conductor, tarjeta de identidad, pasaporte

• Numbero de Seguro social para usted y los otros miembros de su familia

• Verificacion de ingresos durante el ultimo mes. Si funciona, vamos a necesitar un sueldo

• Si usted esta Casado necesitamos su trozo, asi

• Si usted recibe manutencion o pension alimenticia, temenos los totals

• Traer cualquier sefuro de salud actual: politica, nombre y numberos de identificacion

• If there are ANY changes to your Medicaid application, please call the number above with in 10 days.

• If you need a copy of your birth certificate and you were born in LA, call or stop in at the Health Unit at (337)480-2670 from 8:00A.M.-3:00P.M.
